The Phillies made the following transactions today:

• Left-hander Cliff Lee was activated from the 60-day disabled list.

• Infielder Reid Brignac was activated from the 15-day disabled list.

• Catcher Wil Nieves was activated from the 15-day disabled list.

• Outfielder Tony Gwynn Jr. was designated for assignment.

• Infielder Cesar Hernandez was optioned to triple-A Lehigh Valley.

• Catcher Koyie Hill was outrighted.
