Community Champion of the Homestand

The UnitedHealthcare Community Champions program honors individuals who make a meaningful and lasting impact across Twins Territory. Each weekend homestand, one nominated individual will be recognized for their positive contributions to community well-being, opportunity, and connection. We are seeking individuals whose leadership, service, and character strengthen the fabric of our communities and inspire others across generations. Anyone may submit a nomination.

What is the UnitedHealthcare Community Champions program?

The UnitedHealthcare Community Champions program (the “Program”) celebrates individuals who are making a meaningful and lasting impact across Twins Territory. Presented in partnership with United Healthcare Services, Inc. (“UnitedHealthcare”), the program recognizes people whose leadership, service, and dedication strengthen communities, expand opportunity, and promote well-being for others.

Community Champions represent the spirit of service that helps communities thrive — individuals who uplift those around them, lead with purpose, and contribute to a healthier, more inclusive future. By sharing their stories, the Minnesota Twins and UnitedHealthcare hope to recognize these contributions and inspire others to make a difference in their own communities.

Who can be nominated as a UnitedHealthcare Community Champion?

A nominee for the Program (a “Nominee”) must be a U.S. resident (including residents of United States commonwealths, territories, and possessions), eighteen (18) years of age or older (except in the case of legal residents of certain states where the legal age is greater than eighteen (18) years of age, such legal age of majority), and an individual who demonstrates exceptional commitment to improving the well-being of others and strengthening their community. Nominees should embody the values of leadership, compassion, and service through meaningful actions that create positive and lasting impact.

Community Champions may contribute through a variety of efforts, including advancing health and wellness, fostering inclusion and opportunity, supporting education or mentorship, strengthening community connections, or leading initiatives that improve quality of life for others. Nominees should demonstrate a genuine dedication to helping others and making their community a better place for future generations.

Who can nominate an individual as a UnitedHealthcare Community Champion?

Any U.S. resident (including residents of United States commonwealths, territories, and possessions) who is eighteen (18) years of age or older (except in the case of legal residents of certain states where the legal age is greater than eighteen (18) years of age, such legal age of majority) may nominate an individual for the Program, provided the nominator has secured permission from the Nominee to disclose their personal information within the Nomination Form (defined below). Self-nominations are permitted in accordance with these guidelines.

All individuals submitting a Nomination Form agree to release and hold the MLB Entities (defined below), including Minnesota Twins, LLC d/b/a Minnesota Twins Baseball Club (the “Minnesota Twins”) and UnitedHealthcare, harmless from and against any and all rights, claims, and causes of action that may arise against either of them in connection with a Nomination Form submitted as part of the Program. “MLB Entities” means the Office of the Commissioner of Baseball (“BOC”), its Bureaus, Committees, Subcommittees and Councils, MLB Advanced Media, L.P., Major League Baseball Properties, Inc., The MLB Network, LLC, the Major League Baseball Clubs (“Clubs”), each of their parent, subsidiary, affiliated, and related entities, any entity which, now or in the future, controls, is controlled by, or is under common control with the Clubs or the BOC, and the owners, general and limited partners, shareholders, directors, officers, employees, and agents of the foregoing entities.

How do I nominate an individual as a UnitedHealthcare Community Champion?

Nominations for the Program are made by completing the online nomination form at twins.com/communitychampion (the “Nomination Form”). Nominations will begin at 11:00a.m. CT on March 31, 2026. All Nomination Forms must be received by 11:59p.m. CT on August 29, 2026.

How will UnitedHealthcare Community Champions be selected?

All nominees will be reviewed and considered by UnitedHealthcare and the Minnesota Twins. A total of twelve (12) nominees will be selected. Final selected Nominees will be selected at the joint discretion of UnitedHealthcare and the Minnesota Twins. Individuals selected as 2026 UnitedHealthcare Community Champions will be notified via the email address provided in the Nomination Form no later than September 5, 2026. If a selected Nominee does not acknowledge acceptance within seventy-two (72) hours of a communication notifying the Nominee of their selection, UnitedHealthcare and the Minnesota Twins reserve the right to select an alternate qualifying Nominee at their discretion.

How will the selected 2026 UnitedHealthcare Community Champions be honored?

Each selected Community Champion receives:

  • Four (4) Champions Club tickets for selected Nominee and three (3) guests to a designated 2026 Minnesota Twins regular season weekend home game;
  • The opportunity for selected Nominee to receive on-field recognition at the game; and
  • A custom plaque in the selected Nominee’s name, displayed within the UnitedHealthcare Champions Club at Target Field for up to two (2) weeks.

If a selected Community Champion is unable to attend the designated recognition game, the Minnesota Twins may provide tickets to an alternate mutually agreed upon Minnesota Twins regular season home game, subject to availability.

General terms - What else should I know?

The Minnesota Twins may contact nominators or selected Nominees using the information provided in the Nomination Form for the purpose of evaluating nominations and notifying selected Nominees. All decisions made by the Minnesota Twins and UnitedHealthcare in connection with the Program will be based on the information provided in the Nomination Form or in any follow-up discussions with a Nominee or nominator, and all such decisions are final. No correspondence will be entered into regarding these decisions. No transfer, assignment, or substitution of any Program benefits will be permitted except at the sole discretion of the Minnesota Twins. Travel to and from Target Field is not included. Use of any Major League Baseball game, event, or exhibition ticket is subject to the standard terms, conditions, and health and safety policies applicable to the ticket; see twins.com/ticketback for details. Seat locations and, unless otherwise stated, the exact game date will be determined at the sole discretion of the Minnesota Twins. Guest(s) of selected Community Champions must be of the legal age of majority in their jurisdiction of residence unless accompanied by a parent or legal guardian. Major League Baseball game dates and times are determined at the sole discretion of the BOC and/or the applicable Club and may be subject to change. The terms and conditions of tickets included as part of the Program will govern in the event a legal game or event, as defined by Major League Baseball, is not played due to weather, an act of God, terrorism, civil disturbance, or any other reason. Community Champions and their guest(s) agree to comply with all applicable ballpark and venue regulations; see mlb.com/twins/ballpark for details. The Minnesota Twins and the other MLB Entities reserve the right to remove or deny entry to any selected Nominee or guest who engages in non-sportsmanlike or disruptive behavior or who acts with intent to annoy, abuse, threaten, or harass any other person, causes damage to person or property, harms the reputation of the Minnesota Twins or MLB Entities, or otherwise violates ballpark policies. The Minnesota Twins and MLB Entities are not responsible for weather conditions, acts of God, terrorism, civil disturbances, government regulations, work stoppages, epidemics, pandemics, or any other issue concerning public health or safety, or any other event outside of their control that may cause cancellation or postponement of a Major League Baseball game or event. Tickets provided as part of the Program may not be resold, offered for resale, or used for any commercial or promotional purpose. Any such resale or commercial use may result in disqualification and forfeiture of Program benefits. The Minnesota Twins and MLB Entities (a) make no warranty, guarantee, or representation of any kind concerning any Program benefit and (b) disclaim any implied warranties. By participating in the Program, nominators and Nominees agree to waive any and all claims against the Minnesota Twins and MLB Entities for any injury, damage, or loss that may occur, directly or indirectly, from participation in the Program or receipt or use of any Program benefit. By submitting a nomination, nominators and Nominees agree to be bound by these terms and conditions. Community Champions, by accepting recognition as part of the Program, grant the Minnesota Twins, the MLB Entities, UnitedHealthcare, and their respective designees the right to publicize their name, likeness, voice, statements, photograph, and related information for advertising, promotional, or trade purposes in any media now known or later developed, worldwide and in perpetuity, without further compensation or notice unless prohibited by law. The Minnesota Twins reserve the right to require selected Nominees to provide documentation supporting the information submitted in their nomination and to complete additional forms, including but not limited to eligibility certification, liability release, publicity release (except where prohibited by law), photo/image waiver, and authorization for a background check. These terms and conditions are governed by and construed in accordance with the laws of the State of Minnesota.
