ENTRY FORM

No purchase necessary. Contest closes 4:59 p.m. ET on June 24, 2024. Open to residents of Canada of age of legal majority. Five (5) prizes to be won, each valued at approx. CAD $5,000 and consisting of a trip for two (2) to Toronto, Ontario to attend one (1) Blue Jays™ home game at Rogers Centre™ stadium on August 7, 2024 (tickets for all prize winners will be located within one (1) Rogers Centre luxury suite), Blue Jays-branded merchandise, one baseball autographed by a Blue Jays player, a CAD $500 prepaid credit card, a dinner at a restaurant selected and hosted by the contest sponsor, and a Rogers Centre stadium tour. If winner resides within 300 km of Rogers Centre, airfare not included in prize and actual prize value will be less than stated amount. Limit of one entry per person/email address with the opportunity to receive ten (10) additional entries per person. Odds of winning depend on number of eligible entries received and whether entrant chooses to receive additional entries. Must correctly answer a mathematical skill-testing question to win.
