How will winners be acknowledged?

The winner of the award will receive four game tickets to a pre-selected game, on-field acknowledgement and the opportunity to throw out a ceremonial first pitch prior to the game, along with a Brewers Community Achiever certificate and a gift bag filled with Brewers memorabilia. The winners' photo and name will be featured on the Brewers website during the season.

How to nominate your Brewers Community Achiever:

Fans can nominate their Brewers Community Achiever online now or pick-up a nomination form at the Guest Relations Booth located on the Field Level Concourse at Miller Park.

Who is eligible to participate?

All adults and school-aged children throughout the State of Wisconsin are eligible to participate in the Brewers Community Achievers Program.

Nomination Form

NO PURCHASE NECESSARY TO ENTER. Open to legal residents of Wisconsin. Void where prohibited.


Milwaukee Brewers Community Relations Department will review nominations on a monthly basis.

The Community Relations department will review all nominations based on their community service/volunteer organization affiliations.

Nominators will be notified via phone or email if their submission was selected as a winner.

A winner will be selected each month from April - September and will be notified via phone or email.


All adults and school-aged children throughout the State of Wisconsin Volunteers, philanthropists and other heroes in the community.
