US Mode Milestones Program

Official Terms and Conditions

GENERAL NOTICE: No Purchase or Sales Presentation Necessary to earn a ticket voucher. Void where prohibited.

TIMING: The US Mode Milestones program (the “Program”) will begin at 9:00 a.m. May 9, 2025, and will end at 11:59PM, CT August 31, 2025, or when ticket supplies are exhausted, whichever occurs first (the “Program Period”). (See early termination details below.)

ELIGIBILITY: Open to legal residents of the 50 U.S./D.C 18 years of age or older (19 or older in AL and NE) who have completed the Screen Free Scorecard, which can be found at www.brewers.com/USMode, and have submitted it in accordance with the instructions herein during the Program Period.

Sponsor: The Program is sponsored by UScellular, 8410 West Bryn Mawr, Chicago, IL 60631.

AWARD: One (1) voucher code redeemable online for one (1) Terrace Level ticket to one (1) 2025 Milwaukee Brewers Monday - Thursday regular season home game at American Field in Milwaukee, WI. Limit one (1) award per phone number for the entire Program Period. See voucher redemption instructions below.
** **HOW TO CLAIM AN AWARD
: Bring your completed Screen Free Scorecard to any participating UScellular store in the state of Wisconsin (https://www.uscellular.com/store-locator) during the Program Period. Present it to a UScellular associate, complete a short digital survey, and your ticket voucher code will be sent to you via the email you provide. Please allow approximately sixty (60) minutes to receive your voucher code.

HOW TO REDEEM AN AWARD: You must redeem your Milwaukee Brewers ticket voucher code at https://www.mlb.com/brewers/tickets/vouchers during the Program Period. NOTE: Receipt of a voucher code does not guarantee that you will get a ticket, as voucher redemption is subject to ticket availability. TICKET QUANTITIES ARE LIMITED. Tickets are distributed by the Milwaukee Brewers on a first-come, first-served basis; therefore, prompt redemption is recommended.

Note: You may also be able to purchase additional tickets for the same game at that time, subject to ticket availability.

TICKET VOUCHER AND GAME TICKET DETAILS: Each Participant is fully responsible for any and all applicable federal, state, and local taxes (including income and withholding taxes). All costs and expenses associated with the Award acceptance and use not specified herein as being provided, including but not limited to lodging, transportation costs, meals, gratuities and other expenses incurred by accepting the Award are the sole responsibility of each individual Participant. TRANSPORTATION TO/FROM AMERICAN FAMILY FIELD AND LODGING NOT INCLUDED WITH ANY TICKET VOUCHER. Use of any Major League Baseball game/event/exhibition ticket is subject to the standard terms, conditions, and health and safety policies applicable to the ticket; see brewers.com/ticketback for details. Seat locations at the game/event/exhibition and, unless otherwise stated in the award description, exact game/event/exhibition date shall be determined in the sole discretion of Sponsor and the Milwaukee Brewers. Participant’s guest(s) must be of legal age of majority in their jurisdiction(s) of residence unless accompanied by a parent or legal guardian. Major League Baseball game/event/exhibition dates and times are determined in the sole discretion of the BOC and/or the applicable Club and may be subject to change. The terms and conditions of the tickets awarded as part of the award will govern in the event a legal game/event/exhibition, as defined by Major League Baseball, is not played or held due to weather conditions, an act of God, an act of terrorism, civil disturbance, or any other reason. Participant and their guest(s) agree to comply with all applicable ballpark and venue regulations in connection with the award; see brewers.com/ballpark for details. Sponsor and the other MLB Entities reserve the right to remove or to deny entry to the participant and/or their guest(s) who engage in a non-sportsmanlike or disruptive manner or with intent to annoy, abuse, threaten, or harass any other person at the game/event/exhibition. Released Parties (as defined below) will not be responsible for weather conditions; acts of God; acts of terrorism; civil disturbances; local, state, or federal regulation, order, or policy; work stoppage; epidemic, pandemic, or any other issue concerning public health or safety; or any other event outside of their control that may cause the cancellation or postponement of any Major League Baseball game/event/exhibition. Major League Baseball game/event/exhibition tickets awarded as awards may not be resold, offered for resale, or used for any commercial or promotional purpose whatsoever. Any such resale or commercial or promotional use may result in disqualification and award forfeiture, and may invalidate the license granted by the game/event/exhibition ticket.

By participating in the Program, you accept and agree to these Official Rules and the decisions of the Sponsor and the Milwaukee Brewers, which shall be final in all matters relating to this Program. The Program is subject to all federal, state and local laws and regulations and is void where prohibited.

RELEASED PARTIES

By participating in the Program, you agree to release and hold Sponsor, the Milwaukee Brewers, the other MLB Entities*, and each of their respective parent companies, subsidiaries and affiliates and the respective directors, officers, employees and assigns of each of the foregoing (collectively, the “Released Parties”) harmless from and against any and all claims and liability for any injury, damage or loss of any kind that may occur, directly or indirectly, in whole or in part, from participation in the Program or from the use or redemption of a voucher code or ticket awarded to him/her, including, without limitation, any claims for property loss, damage, personal injury or death.

*The "MLB Entities" shall mean the Office of the Commissioner of Baseball ("BOC"), its Bureaus, Committees, Subcommittees and Councils, MLB Advanced Media, L.P. ("MLB"), Major League Baseball Properties, Inc., The MLB Network, LLC, the Major League Baseball Clubs ("Clubs"), each of their parent, subsidiary, affiliated and related entities, any entity which, now or in the future, controls, is controlled by, or is under common control with the Clubs or the BOC, and the owners, general and limited partners, shareholders, directors, officers, employees and agents of the foregoing entities.

EARLY TERMINATION

The Released Parties are not responsible for technical, hardware or software malfunctions, lost or unavailable network connections or failed, incorrect, inaccurate, incomplete, garbled or delayed electronic communications whether caused by the user or by any of the equipment or programming associated with or utilized in this Program, or by any human or other error which may occur in the transmission, receipt or processing of any voucher redemptions/ticket Awards in this Program, or in the administration of the Program, generally.

The Sponsor reserves the right, in its sole discretion, to suspend, modify or terminate the Program should any unauthorized human intervention, human, technical or other error, fraud or other issues, including but not limited to issues affecting UScellular’s business operations, corrupt or affect the administration, security, fairness or proper conduct of the Program.

If terminated prior to the stated end date of the Program Period, no further voucher codes or tickets will be awarded. All properly obtained, non-suspect voucher codes and tickets will remain valid.

Disputes/Venue/Governing Law

By participating in the Program, you agree that: (a) any and all disputes, claims and causes of action arising out of, or connected with, this Program or the voucher codes/tickets shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate federal, state or local court located in Cook County, Illinois; (b) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including but not limited to costs associated with entering this Program, but in no event attorneys’ fees; and (c) to the extent allowed by applicable law, under no circumstances will you be permitted to obtain awards for, and you hereby waive all rights to claim, punitive, incidental and/or consequential damages and/or any other damages, other than out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO PORTIONS OF THE ABOVE MAY NOT APPLY TO YOU.

All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Terms and Conditions, or your rights and obligations and the rights and obligations of the Sponsor in connection with the Program, shall be governed by, and construed in accordance with, the laws of the State of Illinois, without giving effect to any choice of law or conflict of law rules (whether of the State of Illinois or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Illinois.
